Telehealth Consent Agreement

Effective date: May 5, 2026
Important. This Telehealth Consent Agreement is between you (“Patient”) and the independent licensed Provider(s) providing clinical services through the Reaux Health Platform. Reaux Health, LLC facilitates the technology infrastructure for these telehealth encounters but is not your healthcare provider.
Section 01

What is telehealth?

Telehealth refers to the use of electronic information and telecommunications technologies to support clinical healthcare, patient and professional health-related education, public health, and health administration. Services may include, but are not limited to:

  • Asynchronous (store-and-forward) consultations via secure messaging and questionnaire review
  • Synchronous video or audio consultations with a licensed Provider
  • Remote patient monitoring and follow-up care
  • Electronic prescription issuance and pharmacy coordination

Through the Reaux Health Platform, most consultations are conducted asynchronously: your Provider reviews your intake information, medical history, and questionnaire responses and determines a treatment plan without a real-time video encounter, unless the Provider determines that a synchronous visit is clinically necessary.

Section 02

Illinois telehealth law disclosure

Reaux Health operates in compliance with the Illinois Telehealth Act (410 ILCS 151/) and applicable Illinois Department of Financial and Professional Regulation (IDFPR) rules governing telehealth practice. Under Illinois law:

  • Providers delivering telehealth services to Illinois residents must be licensed in the State of Illinois
  • A valid provider-patient relationship may be established via telehealth
  • Providers must comply with the same standard of care applicable to in-person consultations
  • Prescribing via telehealth is subject to applicable state and federal law, including DEA regulations for controlled substances

As services expand to additional states, applicable telehealth laws of those states will govern the Provider-patient relationship in those jurisdictions.

Section 03

Potential benefits of telehealth

Telehealth offers the following potential benefits:

  • Convenient access to licensed healthcare Providers without requiring in-person visits
  • Elimination of geographic barriers to specialty care
  • Reduced time burden associated with travel and in-office waiting
  • Continuity of care through ongoing remote monitoring and follow-up
  • Enhanced privacy and comfort for sensitive health discussions
Section 04

Potential risks and limitations of telehealth

You acknowledge and understand the following risks and limitations inherent to telehealth services:

  • Telehealth consultations do not involve physical examination, which may limit the scope of clinical assessment
  • Technical failures, including internet outages or platform disruptions, may delay or interrupt care
  • Electronic communications may, despite reasonable safeguards, be intercepted or accessed by unauthorized parties
  • Telehealth is not appropriate for all medical conditions or emergencies
  • Providers may determine that in-person evaluation is necessary and refer you accordingly
  • Time zone differences or scheduling constraints may affect response times

Your Provider will use professional clinical judgment to determine whether your healthcare needs can be appropriately addressed via telehealth.

Section 05

Privacy and security of telehealth communications

Reaux Health uses a HIPAA-compliant Electronic Medical Records platform (Healthie) for all clinical communications, records storage, and Provider-patient messaging. We have entered into Business Associate Agreements with all applicable vendors. Security measures include:

  • End-to-end encryption for all clinical communications
  • Secure, access-controlled patient portal
  • Role-based access controls limiting PHI access to authorized individuals
  • Audit logging of all PHI access events

You are responsible for maintaining the security of your own device and account credentials. You should use the Platform only on trusted, secure networks and devices.

Photos, videos, and recordings

You may be asked to provide photos, such as a photo of yourself, an injection site, or your medication, as part of your evaluation or ongoing care. Submission of photos is voluntary, but may be necessary for your Provider to make appropriate clinical decisions.

Photos and any video communications are stored in your secure patient record and are subject to our Privacy Policy and HIPAA. We do not use photos or videos for marketing without your separate, written authorization.

If your Provider conducts a synchronous video visit with you, that visit may be recorded for clinical documentation. You will be informed before any recording occurs and have the right to decline.

Section 06

Provider licensure and location

All Providers offering telehealth services through the Platform are independently licensed healthcare professionals holding valid, active licenses in the state(s) in which they practice and in which you are located at the time of service. You agree to accurately represent your location when using the Platform, as Provider licensure requirements are state-specific.

Reaux Health verifies Provider credentials but does not employ, supervise, or direct the clinical decisions of affiliated Providers. Providers exercise independent professional judgment in all clinical matters.

Section 07

Prescription practices via telehealth

Where clinically appropriate, your Provider may prescribe medications via telehealth. You acknowledge that:

  • Prescriptions issued via telehealth are subject to all applicable federal and state laws
  • Certain medications cannot be prescribed via telehealth (e.g., Schedule II controlled substances without a prior in-person relationship)
  • Your Provider may require additional information before prescribing
  • Issuance of a prescription is not guaranteed and is solely at the Provider's clinical discretion
  • You will use medications only as directed and will not share medications with others

GLP-1 treatment considerations

Reaux Health's affiliated Providers prescribe compounded GLP-1 receptor agonist medications (semaglutide, tirzepatide) for weight management when clinically appropriate. You acknowledge and understand the following considerations specific to GLP-1 therapy:

Eligibility

Treatment is intended for adults who meet clinical criteria determined by your Provider, typically a Body Mass Index (BMI) of 30 or greater, or BMI of 27 or greater with at least one weight-related condition (such as type 2 diabetes, hypertension, dyslipidemia, or sleep apnea). Your Provider will determine whether you qualify based on your full clinical profile.

Contraindications

GLP-1 medications are contraindicated for individuals with any of the following:

  • Personal or family history of medullary thyroid carcinoma
  • Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
  • History of severe or recurrent pancreatitis
  • Gastroparesis or other severe gastrointestinal motility disorders
  • Active gallbladder disease
  • Type 1 diabetes
  • Known hypersensitivity to GLP-1 receptor agonists

You must disclose any of these conditions during intake. Failure to disclose contraindications may result in serious harm.

Pregnancy and breastfeeding

GLP-1 medications are not recommended during pregnancy or breastfeeding. If you are pregnant, trying to become pregnant, or breastfeeding, you must inform your Provider, who will not initiate or continue GLP-1 therapy in these circumstances. If you become pregnant during treatment, contact your Provider immediately.

Potential adverse effects

GLP-1 medications are associated with the following potential adverse effects. By proceeding with treatment, you acknowledge that you have been informed of these risks:

  • Gastrointestinal symptoms (nausea, vomiting, diarrhea, constipation, abdominal pain): most common, particularly during dose escalation
  • Pancreatitis: rare but serious; seek medical attention immediately for severe persistent abdominal pain
  • Gallbladder disease (gallstones, cholecystitis): increased risk reported with GLP-1 use
  • Hypoglycemia: particularly when used with insulin or sulfonylureas
  • Acute kidney injury: typically secondary to dehydration from gastrointestinal symptoms
  • Thyroid C-cell tumors: observed in animal studies; human relevance uncertain
  • Hypersensitivity reactions including rare anaphylaxis

Lab work and monitoring

Your Provider may require baseline and periodic lab work, which may include comprehensive metabolic panel, lipid panel, liver function tests, kidney function tests, and HbA1c, to monitor your safety and treatment response. Failure to complete required lab work may result in treatment discontinuation.

Titration and side-effect reporting

GLP-1 medications are typically initiated at a low dose and increased gradually to minimize side effects. You agree to report any concerning symptoms, particularly severe abdominal pain, persistent vomiting, signs of allergic reaction, or any unexpected symptoms, to your Provider promptly via the patient portal.

Section 09

Compounded medications disclosure

The medications prescribed through the Reaux Health Platform are compounded by state-licensed compounding pharmacies in coordination with your Provider. You acknowledge:

FDA approval status

Compounded medications, including the compounded semaglutide and tirzepatide that may be prescribed through the Platform, are not FDA-approved. While brand-name versions (Wegovy®, Ozempic®, Zepbound™, Mounjaro®) are FDA-approved, compounded versions are prepared by individual pharmacies and have not undergone FDA review for safety or efficacy.

Sourcing

Active pharmaceutical ingredients used by partner compounding pharmacies are sourced from manufacturers registered with the FDA. Compounding pharmacies follow United States Pharmacopeia (USP) compounding standards (USP <795> and USP <797>).

Not equivalent to brand-name medications

Compounded versions are not generic equivalents and may differ from brand-name medications in formulation, excipients, or concentration. They are not direct substitutes for FDA-approved products.

Independent prescribing decision

Your Provider's decision to prescribe a compounded medication is independent and based on clinical judgment. The decision to accept and use a compounded medication is yours, made with your Provider's clinical guidance.

Section 10

Right to withdraw consent

You may withdraw your consent to receive telehealth services at any time by notifying your Provider or Reaux Health in writing via the patient portal. Withdrawal of consent to telehealth will effectively end your ability to receive clinical services through the Platform. Withdrawal does not affect care already received or financial obligations already incurred.

Section 11

Alternatives to telehealth

If you prefer, you may seek the same or similar clinical services through in-person healthcare providers in your geographic area. Your Provider can assist with referrals to in-person care when clinically appropriate or upon your request.

Section 12

Emergency disclaimer

The Reaux Health Platform is not designed for medical emergencies and must not be used as a substitute for emergency care.

If you are experiencing a medical emergency, you should:

  • Call 911 immediately
  • Proceed to the nearest hospital emergency room
  • Contact your local emergency services

Emergency warning signs that require immediate in-person care include: chest pain or pressure, difficulty breathing, stroke symptoms, severe allergic reactions, loss of consciousness, or uncontrolled bleeding.

Section 13

Technical requirements

To effectively use telehealth services through the Platform, you should have:

  • A compatible device (smartphone, tablet, or computer)
  • A reliable internet connection
  • A valid email address for account access and communications
  • Access to the Reaux Health patient portal

Section 14

Acknowledgment and consent

Acknowledgment of subscription terms

By enrolling in a Reaux Health subscription, you acknowledge that:

  • Your subscription includes access to your independent licensed Provider for an initial consultation and ongoing follow-up care for the duration of your plan period (1 month, 3 months, 6 months, or 9 months depending on the plan you select).
  • Medication, when prescribed, is shipped by the partner pharmacy in a single bundled fulfillment after Provider approval, and the entire plan supply is delivered at once.
  • Refunds are available only before pharmacy fulfillment. Once medication has been shipped, your subscription is non-refundable for the duration of the plan period.
  • You may decline or discontinue treatment at any time at your own discretion, and your Provider will support you with clinical guidance if you choose to do so. Discontinuation of treatment does not entitle you to a refund of any portion of the subscription after pharmacy fulfillment has occurred.

Accuracy of self-reported information

You acknowledge that the information you provide during intake, including medical history, current medications, current health status, contraindications, pregnancy status, and your physical location at the time of service, is accurate, complete, and truthful to the best of your knowledge. You agree to update your Provider promptly if any of this information changes during treatment. False, misleading, or incomplete information may result in serious harm and may result in immediate termination of your subscription without refund.

Telehealth acknowledgment

By accepting this Telehealth Consent Agreement electronically through the Reaux Health Platform during intake, you acknowledge and agree that:

  • You have read and understand this Telehealth Consent Agreement in its entirety
  • You understand the nature, benefits, risks, and limitations of telehealth services
  • You voluntarily consent to receive healthcare services via telehealth through the Reaux Health Platform
  • You understand that telehealth consultations may be asynchronous and may not involve real-time video
  • You understand that your Provider is an independent licensed professional, not an employee of Reaux Health
  • You understand that in-person care may be recommended or required in certain situations
  • You understand that the Platform must not be used for medical emergencies
  • You have been informed of your right to withdraw this consent at any time
  • You confirm that you are located in Illinois at the time you receive telehealth services
