You have the right to clinical care delivered by licensed Providers exercising independent professional judgment, meeting the same standard of care applicable to in-person treatment under Illinois law.
Providers determine your eligibility and treatment plan based on your medical history, current health status, and clinical evidence. Reaux Health verifies Provider credentials and licensure, but does not direct or supervise clinical decisions.
You have the right to know your Provider's name, professional title, license number, state(s) of licensure, and qualifications. You may request this information at any time through your patient portal or by emailing contact@reauxhealth.com.
Independent Providers using our Platform are required to hold valid, active licenses in the state where you are located at the time of service.
You have the right to receive clear, honest information about your treatment options, including potential benefits, risks, side effects, contraindications, alternatives, and limitations, in language you can understand.
Your Provider will discuss these with you before initiating or continuing treatment, and you may ask questions at any time through the patient portal.
You may refuse any treatment, decline any recommendation, or discontinue your subscription at any time without judgment or coercion. Your Provider will support you with clinical guidance regardless of your decision.
Discontinuation may be subject to the refund and shipping terms in our Shipping & Returns policy. Once medication has been shipped, your subscription is non-refundable for the duration of the plan period.
You have the right to know the full cost of your subscription up front, including the medication, Provider consultations, supplies, shipping, and any other services covered. We do not charge hidden fees or surprise add-ons.
Itemized billing statements are available on request by emailing contact@reauxhealth.com or through your patient portal. Reaux Health is cash-pay only; we do not bill insurance.
You have the right to understand the expected duration of your subscription (1 month increments) before enrollment, the schedule for medication shipment, and the expected cadence of Provider follow-up.
Your Provider will discuss treatment timelines, dose titration, and follow-up expectations with you during intake.
You have the right to have your Protected Health Information (PHI) and Consumer Health Data handled in accordance with HIPAA, the Illinois Personal Information Protection Act (PIPA), the Illinois Telehealth Act, and applicable state consumer health data laws.

You have the right to request a copy of your medical records, in electronic or paper form, generally within 30 days of your request. You may also request a correction to information you believe is incorrect or incomplete.
Contact our Privacy Officer at privacy@reauxhealth.com to submit a request. A reasonable, cost-based fee may apply for paper copies.
You have the right to communicate with your care team through the patient portal and to receive responses within reasonable timeframes, typically within one business day for non-urgent inquiries.
Your Provider may also schedule a synchronous (real-time) consultation when clinically necessary. The Reaux Health Platform is not designed for medical emergencies; for emergencies, call 911 or proceed to the nearest hospital emergency room.
You have the right to be treated with dignity and respect at all times, regardless of race, color, religion, national origin, sex, gender identity, sexual orientation, age, disability, military or veteran status, marital status, source of income, or any other characteristic protected under applicable federal or Illinois law.
If you experience any conduct from a Provider, Reaux Health staff, or our partners that you believe violates this right, you may report it confidentially to legal@reauxhealth.com.
You have the right to know that your medication is dispensed by state-licensed compounding pharmacies under the supervision of your Provider, that compounded medications are not FDA-approved, and the name of the dispensing pharmacy upon request.

You have the right to file a complaint about your care, your Provider, billing, or any aspect of the Reaux Health Platform without fear of retaliation, denial of service, or interruption of your treatment.
Submit complaints by emailing legal@reauxhealth.com. We will acknowledge receipt within five (5) business days and respond substantively within thirty (30) days.
